Super Boy 4 once again deviates from the usual pattern of the previous three Super Boy games by changing Super Boy's design completely, this time not being based off of Mario. The graphics, however, are still ripped from Super Mario World but with reduced color depth due to the Master System's technical limitations. Despite this, they are an improvement on Super Boy 3's due to the Master System having better graphical capabilities than the MSX or SG-1000.

The sound effects are ripped from Super Boy 3, and some of the songs are reused from Super Boy 3. The game's items, unlike the first three Super Boy games, are not randomized, and are the same regardless of whether you started another game or not. This is believed to be the final Super Boy game.
